How to sign up as a sub-contractor

The launch of Smokefree Berkshire, provided by Solutions 4 Health, commenced on October 1st 2012. This contract commissions 5,000 quitters per year and will provide support for smokers up to three months after they set a quit date. A range of services will be provided, including individual, group and telecare support, in a wide range of community locations, including GP surgeries and Pharmacists.

The opportunities for GP’s and Pharmacists to operate as sub-contractors to Solutions 4 Health are available and the following options are available.

(a) Formal sub-contract

  • Service Level Agreement (SLA) with Solutions 4 Health: annual accreditation and target
  • Service delivery by Practice Nurses or Health Care Assistants
  • Full training and support will be provided by Solutions 4 Health to meet required quality standards, including CO verification
  • Payment by Results tariff
  • Monthly advice notes will be generated from submitted electronic quitter forms to enable invoice generation
  • Sub-contractors to take financial responsibility for pharmacotherapy; quitter tariff is all-inclusive:
  • Direct supply model for NRT; sub-contractors may source themselves or use supply agreements negotiated by S4H
  • Prescription drugs e.g. Champix will be sourced under FP10 arrangements, but costs will be recharged and deducted from quitter payments.

This option is expected to appeal to practices with one of more practice nurses motivated to deliver the service and able to deliver 50 quitters / year to make the management effort worthwhile.

(b) Referral to Solutions 4 Health run clinic

  • Solutions 4 Health will run clinics on surgery premises on a weekly basis at a mutually convenient day and time.
  • Solutions 4 Health will expect a viable number of referrals and will pay a referral fee for each successful 4 week quitter

This option is expected to appeal to practices with large numbers of smokers and clinic space but no appropriate practice nurse to deliver the service.

(c) Referral to service

  • Practices may refer patients to Solutions 4 Health clinics run throughout Berkshire in a range of community settings, including hospitals, shopping and leisure centres.
  • Solutions 4 Health will pay a referral fee for each successful 4 week quitter.

This is the default option. Those choosing this option will be asked to sign a simple service level agreement (SLA) to ensure both parties are aware of the practice.

Note: practices may also claim QOF points for referrals made under each of the three options.
